I have read and understood the Information Notice prepared in accordance with Article 10 of Law No. 6698 on the Protection of Personal Data, titled “Data Controller’s Obligation to Inform,” regarding who will process my personal data and for what purposes, to whom and for what purposes the processed personal data may be transferred, the method and legal basis for personal data collection, and my rights stipulated in Article 11 of the Law. I have been informed in detail about this matter by ÖNMED HEALTH LTD. ŞTİ., the data controller.

By accepting the Information Notice, I consent to ÖNMED HEALTH LTD. ŞTİ. processing my personal data under the Personal Data Protection Law. I hereby consent to my contact information being transferred to ÖNMED HEALTH LTD. ŞTİ. within the scope of the Information Notice. I hereby consent to the processing, storage, sharing, modification, updating, periodically checking, rearranging, classifying, and preserving of my personal and private data provided by my relatives of my choosing, for the purposes of fulfilling my legal obligations stipulated in the regulations in the legislation, protecting public health, carrying out preventive medicine, medical diagnosis, treatment, and care services, planning and managing health services and their financing, carrying out my examination, preventive medicine, medical diagnosis, treatment, care, and control services, improving the medical treatment application applied to me, reminding me of my appointment dates for ongoing treatments, and keeping me personally informed of innovations in medical treatment and practices. I also consent to the recording, processing, storage, modification, updating, periodically checking, rearranging, classifying, and preserving of my personal and private personal data provided by my relatives of my choosing; I EXPRESSLY CONSENT to the transfer of my personal and sensitive personal data to authorized public institutions and organizations to fulfill the purposes described above; to companies from which we receive support services to carry out our activities; and to independent auditing companies within the framework of legal obligations and legal restrictions; to your legal department; and, upon request, to judicial authorities or law enforcement agencies pursuant to relevant legislation; to the transfer of my personal and sensitive personal data abroad to fulfill the purposes described above; and to the sending of SMS, email, and postal messages, as well as mobile or online communication, for the purposes of informing me of innovations and developments regarding the medical services offered, reminding me of appointment dates for ongoing treatments, and for congratulations and celebrations on special occasions, sending me my orders, and providing promotions and information.
